Forms for Faculty Participants
At semester’s end, you may need to submit the following completed forms:
Portfolio Certification Form
Submit one within each portfolio on Portfolio Assessment Day.
Incomplete Grade Form
Submit to the committee chair after the portfolio assessment for any student who fails the portfolio assessment, but earns a passing grade in the course.
Change of Grade Form
Submit copies to Enrollment Services and the committee chair for any student who passes the assessment after revising their portfolio.
